Thank you for taking the time to fill in this enduring permission form for your child/ren to attend Kids Club.
Effective 1st January to 31st December 2020 in relation to Kids club activities. It is to be completed for all children under 18 year

Once this online form has been completed, you will be required to sign a hard copy of this in person. The form will be printed and made available for you to sign on-site at St Stephen's Anglican church on a Friday afternoon at Kids Club or during church office hours.
If you are unable to come in to sign the form, one of the Ministry Team are happy to organise to call by and visit you and get your signature.

The safety and care of your child are of utmost importance to us, hence we do require a hard-copy, in-person signature for all U18s involved in activities related to our church. Thank you for your understanding with this.

In accordance with the Privacy Act, 1988, we inform you that all personal information collected will be used for the purpose of contacting carers in the event of an accident/emergency, supplying appropriate medical care for your child if the need arises and for informing you of future children’s activities that are authorised or approved by Newtown Erskineville Anglican Church. It will not be distributed to any other organisation without consent.
Thank you for providing this important information. The safety and well being of the children is our primary concern.

Authorisations and Expectations

I give permission for my child to attend all scheduled Kids club activities, unless I advise the kids club leaders otherwise.
I give permission for my child to travel in a car driven by an approved Kids leader or a parent approved by a Kids club leader unless I advise the kids club leaders otherwise. (Your child will not be in a car driven by a learner or provisional licence holder).
I authorise the Kids clu b leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.
I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of Kids club.
I will provide the Kids club leaders with any information relevant to the wellbeing of my child prior to him or her attending a Kids club activity.
I confirm that the information given in this form is true and correct, and will advise Kids club h Leaders of any changes to this information.
